We only see a close-up of Forsythe's gun about to fire; the shooting is referred to afterwards, but it's not absolutely confirmed (just strongly implied) that she was killed. (Nudity alert: Topless)
Against the Law (1997) [Officer Lustig]: Shot three times in the chest by Richard Grieco in a shoot-out at a gas station.

Citizen Toxie: The Toxic Avenger IV (2000) [Woman who Falls from High Rock]: Pushed off of a high rock and into a grotto by the Noxious Offender in a black & white newsreel-parody sequence. (Note: While this fall isn't confirmed as being fatal, and doesn't even appear to be a dangerous distance, I thought I'd go ahead and list it, considering that the rest of the "newsreel" is a massacre.)
