Barbara Shelley (1933 - )
Cat Girl (The Cat-Woman) (1957) [Leonora Brandt, a.k.a. Mrs. Richard Johnson]: Reportedly run over by a car while in her panther form; her body is shown afterwards lying partially under the car, after resuming her human form in death. (I haven't seen this movie myself.) (Thanks to Ezeckial)

The Gorgon (1964) [Carla Hoffman]: Reportedly decapitated while in her monstrous Gorgon form. (I haven't seen this movie myself.) (Thanks to Garrett)

Dracula: Prince of Darkness (The Bloody Scream of Dracula; Disciple of Dracula; Revenge of Dracula) (1966) [Helen Kent]: Bitten on the throat and drained of blood by Christopher Lee; we only see Christopher covering her with his cape as he corners her. She later returns to life as a vampire, and dies once again when Andrew Keir drives a wooden stake through her heart. (Thanks to Garrett)

Rasputin: The Mad Monk (1966) [Sonia]: Reportedly commits suicide by slitting her wrist while under the hypnotic influence of Christopher Lee. (I haven't seen this movie myself.) (Thanks to Garrett)

Blake's 7: Stardrive (1981) [Dr. Plaxton]: Reportedly killed by a power surge when she activates the Stardrive. (Paul Darrow had arranged for the Stardrive to activate as soon as she made the final connection, reasoning that she couldn't have gotten to safety anyway.) (I haven't seen this episode myself.) (Thanks to Garrett)
