Under Two Flags (1936) [Cigarette]: Reportedly shot to death while riding on horseback. (I haven't seen this movie myself.) (Thanks to Gerardo)

Blithe Spirit (1956) [Ruth Condomine]: Killed in a car crash (presumably off-screen) after Lauren Bacall's ghost tampers with the brakes in an attempt to kill Noel Coward. She appears as a ghost throughout the rest of the story. (I haven't seen this version, but I have seen Constance Cummings' death in the 1945 version.)
