Mary Tamm in 'Tales that Witness Madness'
Mary Tamm (1950 - 2012)
Tales that Witness Madness (1973) [Virginia 'Ginny' Pageant]: Stabbed in the back by Michael Petrovich in a shower stall, after he hypnotizes her into a trance. (Nudity alert: Topless & rear)

Doctor Who: The Androids of Tara (1979) [Romana/Princess Strella]: Playing a quadruple role as "Romana," "Strella," and their robot duplicates, the robot Strella's head is bashed in by Tom Baker, and the robot Romana is shot in the back with an energy beam by K-9. (The original versions of both survive the episode.) (Thanks to PortsGuy)

Doctor Who: Destiny of the Daleks (1979) [Romana]: Regenerates (off-screen) by an act of will, transforming into Lalla Ward (after a few rejected forms). (Mary does not appear in this episode; the regeneration begins before the start of the episode.) (Although the Time Lords' regeneration is usually brought about by a fatal injury, such was not the case this time. Still, since I've listed the various Doctors' regenerations as the "deaths" of their incarnations, I'll do the same here.) (Thanks to PortsGuy)

Three Kinds of Heat (1987) [Piou]: Machine-gunned across the chest and stomach by Sylvester McCoy.
