Sian Thomas (1953 - )
Shakespeare: The Animated Tales: Othello (1994; animated) [Desdemona]: Suffocated by Othello (voiced by Colin McFarlane); she dies shortly afterwards while talking to Emilia (voiced by Dinah Stabb).

Sredni Vashtar (2003) [The Aunt]: Throat torn out (presumably off-screen) by the ferret when she goes to find out what's inside the hutch. (I haven't seen this short film, but I'm familiar with the original story.)

Perfume: The Story of a Murderer (2006) [Madame Gaillard]: Throat slit by two robbers in the street. (Thanks to Sheena)

Harry Potter and the Half-Blood Prince (2009) [Amelia Bones]: Although Sian does not actually appear in this film, it's established that her character from the 2007 film Harry Potter and the Order of the Phoenix was killed with the Avada Kevadra curse by Ralph Fiennes. We learn of her death from a newspaper report that Emma Watson reads. (Thanks to Ravenclaw)
