Veronica Lake (1919 - 1973)
a.k.a. Constance Keane
Ex-Mrs. Andre De Toth
I Married a Witch (1942) [Jennifer]: Executed (off-screen) by being burned at the stake; the movie opens with a shot of the burning stake after her death. She returns to life in the 20th century, and later dies in Fredric March's arms when her spirit leaves her body to accompany the spirit of her father (Cecil Kellaway). She later comes back to life yet again when she returns to her body after tricking Cecil's spirit into a bottle.

So Proudly We Hail! (1943) [Lt. Olivia D'Arcy]: Reportedly killed in an explosion when she sacrifices herself by going into the Japanese soldiers' camp with a concealed grenade. (I haven't seen this movie myself, but have read about Veronica's death scene in an article on her career.)
